This is a wonderful image. I think it might benefit if you cropped out most of the top of the fluid bag and the forgrund up to the footboard of the hospital bed. The design on the wall to the ceiling and the metal part of the bed are unnecessary for rhe meaning of the image and their complexity, I feel is also a distraction.

I don't think that the photograph quite says what you feel. I have kept coming back to it but still couldn't figure out why I feel that it doesn't say what you feel. I have a suggestion: While I was studying it and your narrative. I remembered two Alfred Hitchcock movies. I have no idea if you are familiar with Hitchcock movies--he was a celebrated director of mystery and terror movies. One of his visual devices was to photograph a victim or a building in which a victim is about to be murdered by moving away from the building or victim (see the movie "Frenzy".
My suggestion is to go back to the site get close to the bulldozers and use the widest angle lens you have, with the building far away, but not cropped. Take a lot of pictures from all different angles especially some from a low vantage point. In post processing play with lighting adjustments. May be the machines in profile or in sharp light focus.
The bushes in the fodreground, I thnk are distractions. The sky is perfect for what I think you want, but maybe harsh sunlisghts especially in Notrh country light might be even better.
