This is just so much fun and you must have had a lot of fun doing it. It must have taken ages to set it up.
Well thought out and well executed. The composition and colouring are excellent - maybe leaving the plaque there would have been a good pointer for the odd diet!

You've worked it very well. And maybe a different background may help, but it works just as it is.
The pop up background is a useful tool.
My only small problem is the direction of the water (tea) flow. It would look better if there wasn't so much of a curve as it enters the top cup and more of a straight drop from the top cup to the lower one.
It's so easy to overdo an image, so putting the other elements in could have easily spoiled it.

I agree with Denise, that the whites on the right hand side meed toning down a bit - maybe a vignette would do it.
I've used Lens Correction on a duplicated layer in Luminosity mode in an attempt.

Adrian Sommeling looks amazing.
I wasn't too happy with the shadows either, mainly because the characters were photographed without strong lighting - maybe I should have created a backlight and painted shadows coming towards the foreground.
I use a separate layer to paint the shadows, which makes them easier to modify.
